Living Well with Chronic Conditions is a six-week evidence-based group education program that teaches people practical skills they need to live well with a variety of health conditions. Through weekly sessions, the workshop provides support for continuing normal daily activities and dealing with emotions that arise from having a chronic condition.

Requirements
- Anyone living with a chronic condition (examples: asthma, COPD, arthritis, depression, diabetes, cancer, heart disease, obesity and fibromyalgia)
- Family members, friends or caregivers of an individual with a chronic condition

- Living Well with Chronic Conditions is a fun skill-building program designed for persons with chronic conditions
- The program helps individuals take day-to-day responsibility for the care of their chronic condition
- Participants gain skills necessary to self-manage their condition and work effectively with their health care providers

- Techniques to deal with problems experienced by persons dealing with one or more chronic conditions such as frustration, fatigue, anger, pain and isolation
- Appropriate exercise for maintaining and improving strength, flexibility, and endurance
- Appropriate use of medications
- Communicating effectively with family, friends, and health professionals
- Healthy eating
- Managing stress and relaxation
