Adult Information
The three forms
listed below are completed for each adult participant. The Family Record is completed
at the time of enrollment (entry) and updated at program graduation (exit). The
Food Recall and EFNEP Survey are completed at enrollment and graduation.

1. Adult Enrollment Form (Family Record) Form 794
The Family Record must be entered in the computer before
the Food Recall and EFNEP Survey. |

2. Homemaker's 24-Hour Food Recall (Form A)
The initial Food Recall should be completed during the first lesson or shortly
thereafter. It should be entered in the computer as soon as possible so that the
print out can be used for participant instruction. |

3. EFNEP Survey The initial
EFNEP Survey should be completed during the first lesson or shortly thereafter.
It should be entered in the computer as soon as possible so that the print out

Maternal and Infant
Information Data on prenatal, delivery and infant feeding
can be entered into the EFNEP Reporting System 4.0 and later versions. This is
information collected in addition to the Family Record, 24-Hour Recall and EFNEP
Survey. The decision to collect this information is based on how much teaching
occurred in maternal and infant nutrition. Therefore, it is not required by the
state office. But, it does provide very useful impact data to describe educational
efforts in maternal and child nutrition. If you use curriculum targeted to pregnant,
lactating or women who have recently delivered, such as Smart Choices,
you should submit data through ERS. Prenatal

Youth Information
Data on youth
may be entered in individual or group enrollment. |

or you would like to keep addresses and other data on each participant. |

entered into the Youth Group database. (Group enrollment is usually used for groups
of limited duration or when detailed data on participants are not available.) |

Interagency Cooperation
Data
The
Interagency Cooperation form is used to capture information on how EFNEP works
with other programs or agencies. Each county is required to submit data with the
